2. Complete the EOI form.

You can complete the EOI online through an accessible PDF (insert button to click on EOI - PDF accessible)

Or you can download and print an RTF version of the EOI form. ( Insert button to click on EOI - RTF)

3. Return the completed EOI to us.

Email – If you have completed the PDF form on your computer click the submit button – this will prompt you to return the form by email.

If you have completed the RTF form save a copy and email to us.

Post - You can post your EOI to us - attention to Dr Lisa Stafford, GPO Box 2434, Brisbane QLD 4001.
